Our case management team connects participants with healthcare from the community health centers at Philadelphia FIGHT, which are free of charge and do not require insurance to see a health care provider.

Community Health Centers

John Bell Health Center

Provides comprehensive, culturally competent primary medical care to adults age 18 and older.

Y-HEP Adolescent and Young Adult Health Center

Provides inclusive and accessible healthcare for all youth (ages 13–24). Our services include: primary care, mental health care, sexual & reproductive health care and PrEP.

Pediatric and Adolescent Health Center

Offers high quality, accessible, primary care to Philadelphia children and youth from birth through age 18.

FIGHT Family Dentistry

Specializes in a trauma-informed approach to dental care and welcomes patients with dental anxiety or a history of infrequent dental visits. We also coordinate with Philadelphia FIGHT medical providers and case management to ensure priority scheduling for existing FIGHT patients.

Diana Baldwin Mental Health Clinic

Provides culturally competent behavioral health services that address the psychological, social and emotional needs of every patient.

BenePhilly

Offers FREE one-on-one enrollment support to help Philadelphia residents apply for over 20 public benefits. Trained counselors assist with completing applications, organizing key documents, and tracking their applications’ status.
